Overview
Develop and oversee case management for youth assigned to the Thrive Newcomer Program case management team. Provide case management leadership and mentoring to team members. Engage youth with inclusive, culturally competent support for newcomer and immigrant youth. Assess youth needs, discuss options, and develop individualized case plans reflecting strengths and lived experiences. Assess risks and develop safety plans in collaboration with youth, including relational, mental health, substance use, and immigration concerns. Coordinate proactive risk mitigation strategies with the Thrive team. Maintain up-to-date, accurate, and professional case documentation aligned with OCSWSSW standards and ethical guidelines. Ensure documentation reflects client-centered, strengths-based approach, confidentiality, informed consent, transparency, accountability, and professional integrity. Provide referrals to settlement services, immigration legal supports, language access resources, and trauma-specific services. Facilitate access to internal and external service providers (physical/mental health, substance use, housing, immigration pathways such as legal aid, refugee/asylum claims, PR applications). Coordinate case conferences and consultations and advocate for youth within systems such as IRCC, Legal Aid Ontario, and settlement agencies. Maintain up-to-date knowledge of community resources for newcomer and immigration-specific supports. Participate in program development, interdepartmental collaboration, supervision, and continuous learning. Contribute to quality improvement initiatives and evidence-based practice.
