Overview

Performs professional social work of advanced complexity. Assists in resolving complex psychosocial problems. Carries out casework and counselling methods. Consults extensively with hospital staff and community agencies regarding psychosocial aspects of patients' situations. Participates in training of Social Work students. Provides brief supportive counselling (e.g., grief, loss, trauma, adjusting to health diagnosis). Provides resource allocation and referrals. Supports clients with financial and housing applications. Works collaboratively with the Ministry of Children and Families on child protection matters. Investigates, assesses, and intervenes as a Designated Responder under the Adult Guardianship Act

Assess, develop, implement, and evaluate therapy care plans for assigned residents (individual and group basis). Provide unit-level therapy program responsibility and oversight. Provide work direction and delegate work to Rehabilitation Assistants and Rehabilitation Assistant/Drivers. Continuously improve quality and safety in accordance with Island Health mission, vision, values, and strategic direction. Travel between multiple sites as required and arrange transportation to meet operational requirements (may require use of personal vehicle)

Provide shelter services including crisis response and co-planning. Assist with daily physical and emotional supports for clients. Respond to overdoses, administer first aid and Naloxone. Accept and make referrals, and register, orient, and advocate for clients experiencing homelessness. Collaborate with the Victoria Cool Aid Society (VCAS) team and engage with external organizations and professionals to support client needs. Work in a consistently fast-paced, high-acuity, low-barrier shelter environment
