Review referral information for program applicants.

Participate in team meetings and provide input to improve services and ongoing program planning.
Document client contacts/interactions in client charts per documentation guidelines.
Enter client interaction data into the COTA Health database per data collection guidelines.
Assist with other program data collection activities as determined by supervisor/team.
Maintain required certifications and up-to-date training.
Adhere to COTA Health policies/procedures and applicable legislation (including Ontario Human Rights Code).
Comply with Health and Safety policies and respond promptly to concerns/incidents.
Work cooperatively with clients, team members, management, other service providers, and external stakeholders.
Maintain confidentiality of clients, employees, and COTA information.

At present, our wide array of services delivers much needed support to over 4,000 people/year who live with mental health challenges, psychogeriatric mental health conditions, acquired brain injuries, developmental disabilities, dual diagnoses and/or the experience of homelessness. Role Overview The Release Planning Case Management Program is designed to provide short-term support to clients as they transition from custody back into the community. Individuals requiring/requesting support beyond the term of this program will be referred on to other community-based mental health services prior to discharge from the Release Planning Case Management Program. About the Team The Release Planning Case Management Program is designed for inmates with a serious mental illness about to be released from the Toronto South Detention Centre. Cota provides services to male inmates on remand (awaiting trial, sentencing or other proceedings) and offenders who are on short-term sentences while they remain at the Toronto South Detention Centre. Primary Responsibilities: The Case Manager’s primary duties may involve, but not necessarily be limited to:
Reviewing the referral information of applicants to the program
Coordinating and participating in screening interviews with applicants and their respective referral sources
Determining eligibility for the program and confirming acceptance of new clients into the program
Developing a collaborative, supportive and therapeutic relationship with each designated client for whom he/she is assigned primary responsibility
(a) post-secondary degree/diploma in a related field plus 2-3 years direct experience relevant to the target population, or (b) BSW, MSW, or other regulated health care professional designation plus at least 1 year direct experience relevant to the target population. Community experience considered an asset. Excellent clinical knowledge and clinical assessment/support skills relevant to the target population. Advanced understanding of Recovery Oriented/Client Centred service delivery principles. Sound knowledge of community resources and ability to search for applicable resources. Ability to design and implement group activities and facilitate group meetings. Advanced crisis management and dispute resolution skills. Excellent interpersonal and communication skills (verbal and written). High level of computer literacy (Word, email, internet). Good judgment; ability to work independently and cooperatively. Ability to recognize safety issues and follow safe work practices. Demonstrated client-centred and customer-focused approach. Ability to solve problems and make recommendations. Effective team player; sensitive to challenges experienced by marginalized populations (poverty, access issues, mental health and addictions). Must have access to a vehicle (own/borrow/rent or use ride services at no cost to Cota). Conduct visits in clients' homes, hostels, hospitals, other community settings, and the Toronto South Detention Centre. Work within a detention centre with all male clients

Coordinating the purchasing of necessary program/activity supplies, and accounts for purchases in accordance with program guidelines
Participating in team meetings, providing input and ideas for improving services, efficiencies and ongoing program planning Documentation and Data Entry:
Documents all service client contacts/interactions in the appropriate client chart according to COTA Health's documentation guidelines
Enters data relating to client interactions into COTA Health's database system according to data collection guidelines
Assists with other data collection activities within the program, as determined by the Supervisor and/or Team Other Responsibilities:
Maintains current certification and/or up-to-date training in identified “Required Training” areas
Adheres to all COTA Health’s policies and procedures and follows established applicable program guidelines, procedures and practices
Adheres to all applicable legislation, particularly the Ontario Human Rights Code
Comply with all Health and Safety policies and procedures to ensure every reasonable precaution is taken for the protection of workers and clients and that health and safety concerns or incidents are responded to promptly
Works cooperatively, respectfully and constructively with clients, Team members, management, other service providers, and external stakeholders
Ensure that the confidentiality of clients, employees and COTA information is always maintained
Undertakes other related and/or delegated duties, as assigned
