The Case Manager is responsible for carrying out case management, psychosocial rehabilitation, crisis intervention and treatment for individuals living with mental illness. This team member has the clinical skills and experience necessary to assess, plan, develop, co-ordinate, and provide treatment by way of rehabilitative and supportive services in a multi-disciplinary environment. The Case Manager develops supports and opportunities for clients to increase or develop new skills needed to support success and satisfaction in their daily living, learning, working and social environments with the least amount of professional intervention required. This team member will be an expert in engaging the community and developing a network of landlords, community service providers and clients in the District of Muskoka.
Candidates must hold a bachelor's degree in a human service or health science discipline, or an equivalent combination of education and experience. At least one year of experience working with individuals with mental illness, developmental disabilities, or substance use issues is preferred.
