The Licensed Practical Nurse provides health education and care within the scope of nursing practice. They work as part of a client-centered interdisciplinary team to promote wellness and manage chronic diseases.
Candidates must be graduates of an approved Licensed Practical Nurse education program and have completed a Nursing Foot Care program recognized by the CLPNM. A minimum of three years' experience in an inner city community health setting is required.
*These hours are dependent on the operational requirements of the organization *Mount Carmel Clinic operates out of multiple locations; you may be required to work in other areas *Be advised this position may be terminated with appropriate notice upon return of the incumbent as per collective agreement

The Licensed Practical Nurse provides health education and care within the scope of nursing practice. As a member of a client-centered interdisciplinary team, the Licensed Practical Nurse provides care to individuals promoting wellness, prevention of disease and/or disability and the management of chronic diseases/conditions. The incumbent works as an advocate on behalf of the client to ensure the appropriate links/referrals to other human services that would enable the client to achieve their health goals.
Education Graduate of approved Licensed Practical Nurse education program Successful completion of Nursing Foot Care program recognized by the CLPNM. Must be a member in good standing with CLPNM
Skills, Expertise and Experience A minimum of three (3) years’ experience working in an inner city community health setting Knowledge and Experience working with Students and/or Volunteers Current CPR Certification at Basic Rescuer Level

Assets Strong working relationships with Community Resources Fluency in additional languages relevant to the client population Experience working with service specific programs including EMR, eChart, and Accuro
