The LPN provides health education and foot care to individuals within a client-centered interdisciplinary team. They focus on promoting wellness, preventing disease, and advocating for clients to ensure appropriate referrals to human services.
Candidates must be a graduate of an approved LPN program, complete a recognized Nursing Foot Care program, and be a member in good standing with the CLPNM. A minimum of three years of experience in an inner city community health setting and current CPR certification are required.

The Licensed Practical Nurse provides health education and care within the scope of nursing practice. As a member of a client-centered interdisciplinary team, the Licensed Practical Nurse provides care to individuals promoting wellness, prevention of disease and/or disability and the management of chronic diseases/conditions. The incumbent works as an advocate on behalf of the client to ensure the appropriate links/referrals to other human services that would enable the client to achieve their health goals.
Education Graduate of approved Licensed Practical Nurse education program Successful completion of Nursing Foot Care program recognized by the CLPNM. Must be a member in good standing with CLPNM
Skills, Expertise and Experience A minimum of three (3) years’ experience working in an inner city community health setting Knowledge and Experience working with Students and/or Volunteers Current CPR Certification at Basic Rescuer Level
