The Social Worker provides diverse client care including community outreach, mental health support, and case management for individuals with acute and complex needs. The role focuses on reducing barriers to care and promoting overall wellness through a harm-reduction philosophy.
Candidates must be registered with the Manitoba College of Social Workers, possess a valid class 5 driver's license, and have at least five years of social work experience. Specific expertise in addictions, mental health, and crisis intervention is required.

Working as a member of the interdisciplinary Community Wellness team, the Social Worker provides a diverse range of client care to ensure client engagement and to reduce or remove barriers. This care includes extended community outreach, mental health support, health education, case management, treatment support, counseling, community programming, and advocacy for clients with acute and complex care needs. The Social Worker provides this care to individuals while promoting overall wellness and adopting a long-term approach to client care consistent with the philosophy of harm-reduction.
