The Primary Health Nurse II provides health education and care to individuals to promote wellness and manage chronic conditions. They act as a client advocate to ensure appropriate referrals to human services to achieve health goals.
Candidates must be a Registered Nurse in good standing with CRNM, preferably with a Bachelor of Nursing. A minimum of five years of nursing experience and three years in a community health setting are required.

$42.985 - $52.732/hr

*Mount Carmel Clinic operates out of multiple locations; you may be required to work in other areas

The Primary Health Nurse II provides health education and care within the scope of nursing practice. As a member of a client-centered interdisciplinary team, the Primary Health Nurse II provides care to individuals promoting wellness, prevention of disease and/or disability and the management of chronic diseases/conditions. The incumbent works as an advocate on behalf of the client to ensure the appropriate links/referrals to other human services that would enable the client to achieve their health goals.
