As an integral member of the Primary Health Care Team, this individual will use the customer-centered approach to facilitate behavioral health changes through screening, assessments, brief interventions and education. A primary function of this individual will be to hear the customers voice and assist them in achieving their health needs through the development of strong relationships and individualized holistic care plans. Working in a multidisciplinary environment including, but not limited to, Primary Health Care Team members: Family Physicians, Nurse Practitioners, Primary Health Care Case Manager, Home Care, Emergency Medical Services, Pharmacy, Therapy Services, Public Health, Dieticians, psychiatrists, Psychologists, Social Workers, Addiction Workers, and visiting specialists as this position requires networking with other professionals and agencies within the community.
