The Nurse Practitioner will utilize advanced practice skills within a specialized interdisciplinary Stroke Prevention Clinic to provide assessment, coordination, and services, including crisis intervention and short-term management for clients/families. Key duties involve developing and operating the clinic, promoting stroke prevention best practices, triaging referrals, and managing pharmacotherapy for vascular risk factors.
Candidates must possess a Master of Nursing Degree, NP – Adult, and RN(EC) registration in good standing with the College of Nurses of Ontario, along with a minimum of three years of experience in geriatrics and/or community nursing services. Required knowledge includes geriatric psychiatry, relevant legislation, Canadian Stroke Best Practice Recommendations, and proficiency in program development and clinical consultation for the complex frail elderly.
Development and ongoing evaluation of the Stroke Prevention clinic Responsible for the day to day operations of the clinic Making service delivery/program changes in response to volumes and client needs Promoting stroke prevention best practices in the care of client that have experienced a suspected transient ischemic attack (TIA), minor non-disabling stroke, and/or have vascular risk factors. Triaging urgency of referrals using professional judgment and coordinating diagnostic and laboratory testing that are within the RN (EC) scope of practice by utilizing medical directives. Provide pharmacological interventions that include prescribing medications as per RN (EC) scope of practice and by implementation of medical directives. Initiate, renew, titrate and discontinue pharmacotherapy as appropriate to ensure vascular risk factors for stroke are well managed meeting stroke prevention guideline targets. Will evaluate outcomes of individual patient care in collaboration with the interdisciplinary team. In keeping with professional standards of practice, obtains and/or disseminates only that information which is relevant to meeting the client’s identified needs and with appropriate legal authorization, either in the form of signed consents or direct authorization from the client or designated substitute decision-maker. Utilizes expert communication skills by being a link between the client, consultants, resources and the referral source which may be required to coordinate the provision of service to the client. Work in a manner to ensure safety to oneself, clients and their families, colleagues and the work environment. Participates in reflective practice as per identified college standards
Master of Nursing Degree NP – Adult, RN(EC), Registration in good standing with the College of Nurses of Ontario Member of the Registered Nurses Association of Ontario preferred Minimum 3 years experience in geriatrics and/or community nursing services. Knowledge of geriatric psychiatry, community resources, crisis work and a working knowledge of the Regulated Health Profession Act, Mental Health Act, Health Care Consent Act and Substitute Decisions Act. Knowledge of the principles of adult education Demonstrated experience and proficiency in the provision of effective clinical consultation for the complex frail elderly. Strong interpersonal communication skills and ability to establish positive relationships with all staff, clients, families, and community agencies. Understanding of Canadian Stroke Best Practice Recommendations (CSPR). Excellent problem solving ability and critical thinking skills and able to work independently in collaboration with interdisciplinary team Experienced in Program Development and evaluation and analysis of statistics Experience in Change Management an asset. Ability to adapt positively to changes. Excellent verbal and written presentation skills Proficiency with computer applications including Microsoft applications Strong knowledge of Quality improvement processes and LEAN process improvement Excellent attendance and discipline free record required.
