July 2, 2026 Location: Toronto, ON - The Path Home - Pine Villa Compensation: $53,776 - $55,109 Annually Prorated Work Type: Onsite Job ID: 10862279 Existing or New Position: Existing Job Summary: The Personal Support Worker (PSW) is responsible for rendering a range of services for the clients of the program consistent with its basic values and goals. The PSW will support, as assigned, the needs of the vulnerable, at risk, older adults and seniors living in the multi-sites under the auspices of the program. This support relates to the physical, cognitive, mental health and social/recreational needs of the clients. The staff member will maintain the established routines of the program including all security, fire and general health and safety practices. The PSW will perform these and other delegated responsibilities to ensure the quality of personal care of the clients of the program.
The Path Home – Pine Villa is a short- term transitional housing program for hospital patients who no longer require acute care but still require care and support in preparation for a long-term living arrangement. Pine Villa specializes in supporting older adults (55+) with complex physical health challenges, mental health challenges, addiction or cognitive impairment. The Path Home – PIne Villa, provides client centered and goal-oriented support services that enable clients to continue their recovery in the community as they wait for permanent housing.

Coverage: Work rotating shifts, provide coverage as needed, and support overall program operations, including during the absence of senior staff. Additional responsibilities may be assigned by the Program Manager and/or designated supervisor to support overall program operations and continuity of care.
What
You
Bring: Post-secondary education in Human Services or a related field (e.g., PSW Certificate, Gerontology Diploma). Experience supporting seniors and vulnerable populations, including individuals with cognitive and mental health needs. Knowledge of medication administration, health conditions, and psychiatric care. Current First Aid and CPR certification. Experience coordinating social and recreational activities is an asset. Strong skills in assessment, problem-solving, and crisis intervention. Ability to engage clients in a positive, respectful, and supportive manner. Ability to work collaboratively within a multidisciplinary team. Effective verbal and written communication skills. Demonstrated creativity, flexibility, and adaptability. Proficiency with basic computer applications. A valid Ontario Driver’s License is preferred.
Work
Environment: Physically able to perform demanding body mechanics (e.g. lifting, transferring, etc.). Since the position involves working in clients’ own homes, staff may be subjected to second hand smoke while performing their job duties. Staff will be required to work rotating shifts as scheduled by the program.
