Requires a post-secondary education in health care or social services with at least three years of experience in the mental health sector. A valid Ontario driver's license and access to a vehicle are mandatory requirements.

Job Summary: The Case Manager on the M-DOT Hotel Team provides direct service to homeless individuals living with mental health, health, and addictions issues. The position provides both on site and community-based assessment and case management services to individuals living in various physical distancing shelter hotel programs through the city of Toronto. Case Managers work as part of a team that includes Team Leaders, Peer Support Workers, and Housing Support Workers, to meet clients’ housing and other social and health needs. The program works closely with nurses and physicians from Inner City Health Associates, to assess, plan, treat and support clients. The position will involve driving and a valid Ontario driver’s license and insurance are required. Access to a car is a requirement.
