The Primary Health Nurse II provides health education and care within the scope of nursing practice as part of a client-centered interdisciplinary team. This role involves promoting wellness, preventing disease or disability, and managing chronic conditions while advocating for client needs.
Applicants must be graduates of an approved Registered Nursing education program, preferably with a BN, and hold current CRNM membership in good standing. A minimum of five years of experience as a Registered Nurse, including three years in a community health setting, is required.

The Primary Health Nurse II provides health education and care within the scope of nursing practice. As a member of a client-centered interdisciplinary team, the Primary Health Nurse II provides care to individuals promoting wellness, prevention of disease and/or disability and the management of chronic diseases/conditions. The incumbent works as an advocate on behalf of the client to ensure the appropriate links/referrals to other human services that would enable the client to achieve their health goals.
Education Graduate of approved Registered Nursing education program, BN preferred. Must be a member in good standing with CRNM
Skills, Expertise and Experience A minimum of five (5) years’ experience working as a Registered Nurse A minimum of three (3) years’ experience working in a community health setting Knowledge and Experience working with Students and/or Volunteers Current CPR Certification at Basic Rescuer Level
Critical Skills Proven knowledge and ability in critical thinking, decision-making, problem-solving, and crisis management
Focus on Results Demonstrated ability to prioritize and focus work-related activities to effectively and efficiently achieve service goals and produce expected results/outcomes

Assets Strong working relationships with Community Resources Fluency in additional languages relevant to the client population Experience working with service specific programs including EMR, eChart, and Accuro
