The Primary Health Nurse II provides health education and care within the scope of nursing practice as part of a client-centered interdisciplinary team. This role involves promoting wellness, preventing disease or disability, and managing chronic conditions while advocating for client referrals to necessary human services.
Applicants must be a graduate of an approved Registered Nursing education program, preferably with a BN, and hold a minimum of five years of experience as a Registered Nurse, including at least three years in a community health setting. Essential qualifications include proven critical thinking skills and a strong commitment to organizational values like diversity, equity, and cultural safety.

The Primary Health Nurse II provides health education and care within the scope of nursing practice. As a member of a client-centered interdisciplinary team, the Primary Health Nurse II provides care to individuals promoting wellness, prevention of disease and/or disability and the management of chronic diseases/conditions. The incumbent works as an advocate on behalf of the client to ensure the appropriate links/referrals to other human services that would enable the client to achieve their health goals.
Education Graduate of approved Registered Nursing education program, BN preferred. Must be a member in good standing with CRNM Skills, Expertise and Experience A minimum of five (5) years’ experience working as a Registered Nurse A minimum of three (3) years’ experience working in a community health setting Knowledge and Experience working with Students and/or Volunteers Current CPR Certification at Basic Rescuer Level Critical Skills Proven knowledge and ability in critical thinking, decision-making, problem-solving, and crisis management Focus on Results Demonstrated ability to prioritize and focus work-related activities to effectively and efficiently achieve service goals and produce expected results/outcomes Alignment with Organizational Values Commitment to the principles that guide Mount Carmel Clinic, as reflected in the history, vision, mission and values Indigenous Understanding Knowledge and understanding of Indigenous culture and traditions and the impact of colonization on the health and well-being of Indigenous individuals and families Interdisciplinary Team, Collaboration Proven ability to work within an interdisciplinary team environment with respect, professionalism, cooperation, sensitivity and cultural awareness MCC Philosophies Strong knowledge, commitment and demonstrated ability to apply diversity, equity, cultural safety, pro-choice, social determinants of health, population health, harm reduction, trauma-informed and community development principles in all aspects of the work Communication Excellent verbal and written communication skills and proficiency in the use of computer systems, including Microsoft Office suite Assets Strong working relationships with Community Resources Fluency in additional languages relevant to the client population Experience working with service specific programs including EMR, eChart, and Accuro All positions are subject to Criminal Record, Child and Adult Abuse Registry checks.
