Job Description

Reporting to the Clinical Manager, the Clinical Case Coordinator is a member of the interprofessional clinical team. This role provides comprehensive case management services to youth admitted to SAYC throughout their stay. The Clinical Case Coordinator supports youth during admission and identifies and coordinates internal and external resources for youth and their families. In addition, the Clinical Case Coordinator collaborates with the Intake Committee to coordinate the intake process. The hourly rates for this position are: Step 1: $ 35.43 - Step 2: $37.19 - Step 3: $39.06
