  • Document assessments, service plans, progress notes, and case reviews.
  • Manage program waitlists and triage based on acuity, risk, and service priorities.
  • Participate in regional planning initiatives or interagency committees to strengthen cross-sector collaboration and address service gaps

Requirements summary

  • Post-secondary education in Social Work, Autism and Behavioural Science, Psychology, Sociology, or a related human services field.
  • College Diploma in Social Service Work or Developmental Services Work may be considered.
  • Registration or eligibility for registration with a regulated health profession is considered an asset.
  • Relevant practical experience in case management, developmental services, or children’s mental health is required.
  • Valid Ontario driver’s license, reliable vehicle, and automobile insurance including business use coverage with minimum liability of $1 million.
  • Ability to travel throughout the Peel and Halton regions as required.
  • Satisfactory Vulnerable Sector Check and pre-employment medical assessment required.
  • Bilingualism (English/French) and proficiency in other languages are considered assets.

Job Description

The Dual Diagnosis Systems Specialist provides enhanced case management and service navigation for youth with co-occurring developmental disabilities and mental health needs. The hourly rates for this position are: Level I:$39.55; Level II:$41.60; Level III:$43.62

Duties & Responsibilities

  • Case Management
  • Conduct comprehensive intake assessments to identify the needs, strengths, and risk factors of youth with co-occurring developmental disabilities and mental health concerns.
  • Develop individualized service and support plans in collaboration with the youth, their family or caregivers, and relevant community partners.
  • Coordinate multidisciplinary input, including psychiatric consultation, psychological assessment, and specialized clinical services as needed.
  • Facilitate case conferences and team meetings with internal and external stakeholders Service Navigation and System Integration

  • A College Diploma in Social Service Work or Developmental Services Work may be considered.
  • Registration or eligibility for registration with a regulated health profession is considered an asset.
