Acts as a hospital liaison to ensure seamless transitions for clients from in-patient programs to community services. Provides clinical treatment planning, therapeutic counselling, and case management within hospital and community settings.
Requires a Master's degree in human services (completed or in progress) and certification in Stage Screening and Assessment. Must have experience in substance abuse counselling and certifications in Non-Violent Crisis Intervention, CPR, and First Aid.
We are seeking a compassionate and skilled Addiction Counsellor to join our multidisciplinary team. In this role, you will serve as a Hospital Liaison, working primarily within hospital settings (partnership with Trillium Health Partners) to ensure a smooth transition for clients from in-patient and withdrawal management programs to PAARC community services. You will provide clinical treatment planning, therapeutic counselling, case management, and community education, all while upholding the highest standards of care and professionalism.
We are hiring for one (1) full time permanent role, working out of our Argentia office, in the hospital setting and within the community.

Master’s degree in human services (completed or in progress) from an accredited institution. Certification in Stage Screening and Assessment. Experience in substance abuse counselling, with knowledge of withdrawal management and trauma-informed care. Familiarity with anti-racism, anti-oppression frameworks, and a commitment to these principles. Certifications in Non-Violent Crisis Intervention, CPR, and First Aid. Strong communication skills, both verbal and written. Ability to work effectively as part of a multidisciplinary team.
30 am- 4:30 pm. Flexibility to work outside regular business hours as needed. Travel within Peel Region and the Greater Toronto Area (GTA) to meet clients in various settings. Ability to work in different environments, including hospital settings and community locations. Must be comfortable with extensive keyboarding and use of office technology.
