Provide specialized case management and supportive counselling for individuals using opioids, focusing on assessment, treatment planning, and resource connection. Manage a defined caseload through community-based visits, crisis intervention, and collaborative care with other providers.
Requires a Bachelor's degree in a relevant field, a valid Ontario Driver's License with a reliable vehicle, and current CPR/First Aid certification. Candidates should possess evidence-based counselling skills and experience working within a harm reduction framework.
As an Addiction Counsellor/Case Manager (Narcotic Treatment Program - Central West), you will be providing specialized services to individuals (age 16 and older) using opioids. Clients may be considering or utilizing replacement therapies and those seeking to reduce or stop using opioids. As a Counsellor/Case Manager for the Narcotic Treatment Program - Central West, you will primarily focus on case management including assessing needs, connecting clients with resources and supporting them through their goals related to their opioid use.
We are hiring for one (1) full-time, permanent role working within the community (60-70% community based)
