The Occupational Therapist provides direct care, consultation, and capacity enhancement for older adults with complex mental and physical health challenges within the Hospital to Home program. Responsibilities include developing treatment plans, implementing rehabilitation activities, and coordinating care with an interdisciplinary team and community partners.
Candidates must hold a Master's degree in Occupational Therapy and be registered in good standing with the College of Occupational Therapists of Ontario. A valid driver's license or ability to travel across the GTA is required, along with a satisfactory vulnerable sector check.

The Occupational Therapist is part of an interdisciplinary team that provides consultation, direct care, capacity enhancement and support to older adults (and their partners in care), living in the community. The Occupational Therapist is expected to ensure the safety and security of the client(s) environment, to practice in accordance with the College of Occupational Therapists of Ontario standards, and the policies and procedures as defined by LOFT Community Services. The Occupational Therapist will be responsible for working with individuals from diverse backgrounds, contributing to the LOFT Program development, and providing support to the physical, cognitive, mental health and social/recreational needs of older adults and seniors in a way that enhances their dignity and personal empowerment.
