The role involves providing individualized hospice and palliative care to promote resident comfort and quality of life, focusing on pain and symptom management, psychosocial support for residents and families, and expert assessment and care planning. Responsibilities also include collaborating with the interdisciplinary team, which features a strong volunteer component, and performing ongoing assessments.
Candidates must be a Practical Nurse graduate with current, good-standing registration with the College of Nurses of Ontario. Preferred qualifications include demonstrated experience in hospice palliative care, expertise in pain and symptom management, and a minimum of one year of palliative care experience.
St. Joseph's Hospice aims to create a community that honours people as they journey to life's end and provide a haven where the palliative, the caregiver, and the bereaved experience compassion, hope, and healing. Our ten resident suites offer warm and inviting spaces where friends and family can gather together and spend meaningful time in the last hours, days, or weeks of their loved ones' lives. Hospice provides wrap-around care for the palliative individual and their family, supporting physical, emotional, and spiritual needs.
The compassionate, patient-centred care that Hospice provides is made possible by a multidisciplinary team of nurses, personal support workers, counselors, spiritual care providers, housekeeping staff, and a chef, supported by a group of loyal and empathetic volunteers.
The successful candidate will be eligible to join the Hospice Benefits Plan after 3 months of service and as per plan parameters, and the Pension Plan, HOOPP, from first day of employment on a voluntary basis.
About the Role: This is a Regular Part Time position
$30.91 to $31.49 per hour
You will be responsible for providing individualized hospice and palliative care to promote comfort and quality of life for residents in your care. Responsibilities include pain and symptom management, psychosocial care for the resident and family, expert assessment and care planning, and working in partnership with an interdisciplinary team that includes a strong volunteer component. The shifts are 12 hours which includes days, nights, weekends and holidays in a waterfall schedule.
