Provide urgent case management and crisis response for clients seeking walk-in services for addiction and mental health. Assist clients in navigating community resources, designing rehabilitation plans, and accessing essential benefits and services.
Applicants must be between 15-30 years old, Canadian citizens or permanent residents, and possess a valid Police Vulnerable Sector Check. Post-secondary education in social work, mental health, or addictions is required, along with a valid driver's license and vehicle.

The Canada Summer Jobs (CSJ) Addiction & Mental Health Worker, Transitional Case Management Walk-In Services will assist in functions of the department so as to facilitate smooth delivery of walk-in services to clients seeking assistance with system navigation and immediate needs. Assist with short term crisis management by providing an urgent/emergent response to individuals in need of case management support Engage with clients on a short-term basis to support their immediate needs Identify risk factors related to client and staff safety as well as relapse Assess barriers to successful community living (e.g. treatment options, transportation, finances, etc.) Design rehab plans based on client needs, goals and preferences Assist the client to navigate systems and connect to appropriate resources Assist the client to explore options and consequences of different choices Build on the client’s personal strengths, abilities and resources Help clients work towards their vision of a positive personal future Provide needed information and education about mental illness, coping skills, services, Have knowledge about community resources relevant to clients, including other mental health services, entitlement and benefit programs, (e.g. ODSP, Ontario Works, London Housing, etc.) Assist the client to obtain needed resources by educating and supporting the client to access resources, linking the client to appropriate services, advocating with or for the client when needed Complete required documentation related to client as per CMHA organization standards, discipline-specific guidelines and standards and PHIPA legislation
