As a member of Mental Health and Addiction Services, the Community Mental Health Laison Nurse is an integral member of a multidisciplinary team that will address the issues of complex clients who have a primary diagnosis of a major mental illness. This position will work closely with the psychiatry inpatient unit and the transition team to provide assistance with clients transitioning from hospital back into the SHA housing partnership community beds. Responsibilities include intensive case management and stabilization, medication management, crisis intervention, counseling, goal planning, establishment of community supports through consultation with community agencies, student mentoring, documentation, and client/family education.

Experience 3 years experience in a Mental Health setting.
