- Brampton based Homelessness and Recovery Treatment (HART) Hub.
HART Hubs are community-based care hubs designed to address mental health, addiction and housing needs for people experiencing homelessness. They provide integrated care, offering a wide range of services such as primary care, mental health services, addiction care and employment supports. With targeted community outreach and rapid referral pathways, HART Hubs aim to support individuals with complex needs, helping them recover, achieve housing stability, and inclusion.

Role Overview We are seeking a dynamic and compassionate Community Mental Health Counsellor (CMHC) to be an integral part of the HART Hub Brampton program. The CMHC will offer strengths-based case management from a recovery perspective to support clients of the program to find and maintain safe and affordable housing using a housing first approach. They will be a resource and an advocate in their comprehensive understanding of crisis management and the impact mental health challenges might have on an individual and their overall wellbeing. Their main goal is to ensure that clients receive the best support possible to enable them to access and achieve independence in their home and daily activities.
Duties and Responsibilities
- Liaise with the intake and HART Hub team to support successful transition into supportive housing using a housing first approach
- Support clients’ transition into supportive housing and promote housing stability
- Develop and implement individualized wellness, crisis, and recovery plans
- Conduct comprehensive standardized assessments to enhance clients’ mental and physical health and community integration
- Provide strengths-based case management and assist with developing daily skills
- Facilitate access to holistic, person-centred services addressing social determinants of health
- Regularly assess for risk and respond with mitigation strategies, including crisis intervention and de-escalation supports and connecting to crisis services when appropriate.
- Partner with clients, families, and other health care team members
- Facilitate recovery and/or social-recreational groups as needed, including developing new group materials.
- Maintain a network of community resources and coordinate referrals
- Work collaboratively and independently within a multidisciplinary team
- Maintain accurate records and statistical data in line with agency standards
- Update all appropriate documentation within 24 hours of client contact.
- Actively participate in staff meetings, team huddles, supervisions, case conferences, trainings, and events as required.
- Uphold agency health and safety and client safety protocols
- Promote inclusive, equitable, anti-racist, and anti-oppressive practices
- Comply with PHIPA legislation and demonstrate strong communication and organizational skills
- Perform additional duties as required/delegated by Management
Qualifications
- Post-secondary education in Social Work, Health Services, Psychology or equivalent
- 3 years providing Mental Health case management or equivalent care
- Demonstrated knowledge of serious mental illness, concurrent disorders, and/or dual diagnosis, and experience with assessment
- Experience with using addictions and concurrent disorders assessment tools
- Experience working within the homelessness sector
- Knowledge of evidence-based practice for mental health services
- Excellent assessment, risk management, and crisis intervention skills
- Expertise in collaboration with healthcare services
- Knowledge of MS Suite (Word, Excel, Power Point and Outlook)
- A vehicle, valid driver’s license, and adequate insurance coverage
- Valid Certification in First Aid & CPR
- Training in ASIST, CPI and Naloxone administration
Working Conditions and Requirements
- Monday – Friday, with regular evening and weekends shifts included
- Able to sit/stand for prolonged periods of time (building inspections, site meetings, unit inspections, etc.)
- Able to answer and communicate telephone calls and general computer use
- Able to lift up to 20lbs (product, devices etc.)
- Vehicle with adequate insurance coverage to travel to program sites and throughout the community is required
- Demonstrated experience working within a multidisciplinary team and evidence of an ability to collaborate and partner.
- Working knowledge and/or an understanding of various disciplines; including but not exclusive of mental health, substance use, harm reduction, newcomer challenges and barriers faced by those in marginalized populations.
- Demonstrated cultural competence, ability to work and apply practical understanding of different elements of diversity, equity, inclusion and belonging as well as an understanding of social determinants of health and impact on vulnerable communities.
- Knowledge of a variety of evidence-informed treatment modalities including motivational interviewing and trauma informed practices.
- Experienced at engaging in matters that require sensitivity and empathy across a diverse community, excellent judgement, and discretion; ability to operate with various frameworks including culturally sensitive, anti-racist, anti-oppressive, 2SLGBTQ+ affirming and intersectional.
