The Community Mental Health Clinician provides strength-based case management and counseling to support clients with mental health challenges in achieving independence. They collaborate within an interdisciplinary team to implement wellness plans, manage crises, and facilitate access to community resources.
Reporting to the Program Manager, the Community Mental Health Clinician will be responsible for supporting and counselling clients experience mental health challenges in the community. The CMHC will offer strengths based case management from a recovery perspective as a part of a collaborative and interdisciplinary circle of care.
They will also be a resource and an advocate in their comprehensive understanding of crisis management and the impact mental health challenges might have on an individual and their overall wellbeing. Their main goal is to ensure that clients with mental health challenges receive the best support possible to enable them to access and achieve independence in their home and daily activities.
The CMHC, General Mental Health will report directly to the Program Manager. They will work with both internal and external partners and will interact with all levels within the organization.
They will be a support system, resource, and an advocator for the team and Company.
