Provide assessment, treatment, and counseling for patients with obesity and associated chronic diseases within a regional bariatric program. Foster relationships between professionals and agencies while delivering individual, family, and group therapies.
Requires a Master's and Bachelor's degree in Social Work and registration with the Ontario College of Social Workers and Social Service Workers. A minimum of 3 years of experience in chronic disease is required, along with a valid driver's license and vulnerable sector check.

Master’s Degree in Social Work required. Bachelor’s Degree in Social Work required. Current registration in good standing with the Ontario College of Social Workers and Social Service Workers. Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
Recent and relevant experience (minimum 3 years) in the field of Chronic Disease with preference for bariatric experience. Competencies in assessment, treatment and counseling of patients with chronic disease. Experience in community-based, out-patient program or regional program preferred.
