The Case Manager provides individualized support to people with serious mental illness using a bio-psychosocial rehabilitative approach. Responsibilities include developing care plans, providing crisis intervention, and advocating for clients to access community services.
Requires a post-secondary degree in Social Work, Nursing, or Addictions with corresponding professional registration (OCSWSSW, CNO, or CACCF). A valid driver's license and access to a vehicle are mandatory for agency business.
Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; a Registered Nurse or Registered Practical Nurse with current registration with the CNO. (Preference to those with Canadian Certification in Psychiatric/Mental Health Nursing); Bachelor’s Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W; or Bachelor’s Degree with an applicable college diploma in Addictions and current registration with the C.A.C.C.F /or Grandfathered Post-Secondary Degree.
Salary Range: Case Manager Grid HOURS OF WORK: 35 hours per week STARTING DATE: ASAP
This position is responsible for providing individualized and highly flexible support to people who have a serious mental illness utilizing a bio-psychosocial rehabilitative approach to service.
The Case Manager will be required to develop, implement and evaluate individualized plans of care with clients on an ongoing basis.
Reporting to the Manager of Integrated Client Services, the incumbent will be responsible for the following functions: Providing client needs identification through ongoing assessment and client directed care planning Implementing client centered plan of care that focuses on strengths, wellness and empowerment to support recovery and goal achievement Providing crisis prevention and intervention planning with the hope of preventing a relapse and/or hospitalization Acting as a resource person or an advocate to assist individuals in obtaining information/access to services/supports which are deemed needed Liaising with family/significant other and natural supports to facilitate the clients’ links within the community Proficiency in both official languages will be considered an asset.
The candidate will work with other professionals using strong problem solving and communication skills. Demonstrated ability to build partnerships with community partners is required.
A valid driver’s license, and use of a vehicle, which will be used in conducting agency business, is required.
