Reporting to the Manager of Housing and Client Care, the incumbent will be responsible for the following functions: Providing client needs identification through ongoing assessment and client directed care planning Implementing client centered plan of care that focus on strengths, wellness and empowerment to support recovery and goal achievement Working cooperatively with clients and their Clinical Case Manager to address housing needs Developing, seek out and recommend potential market based landlords Implementing client centered approaches to promote housing stability Providing crisis prevention and intervention planning if needed with the hope of preventing a relapse and/or hospitalization Providing short non-intrusive monitoring support Program functions under the Housing First model
Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; a Registered Nurse or Registered Practical Nurse with current registration with the C.N.O. Bachelor’s Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W; or Bachelor’s Degree with an applicable college diploma in Addictions and current registration with the C.A.C.C.F. Experience and a detailed knowledge of serious mental illness, usually gained by several years of working with this population. Valid Ontario Driver’s Licence and access to a vehicle are required.
35 hours/week. Flexible hours are required to meet the position requirements.
Commensurate with experience and education within the range: $57,766-$82,219 inclusive of HOOPP.
