The incumbent is responsible for identifying client needs through assessment and implementing client-centered care plans focused on recovery and empowerment. They will provide crisis intervention and act as an advocate to help clients access necessary community services and supports.
Candidates must hold a post-secondary degree in Social Work, Nursing, or a related field with valid professional registration (e.g., OCSWSSW or CNO). Experience working with Indigenous populations and a detailed knowledge of serious mental illness are required, along with a valid Ontario Driver's Licence.

Post-secondary degree in Social Work from an accredited university with current registration with the O.C.S.W.S.S.W; a Registered Nurse or Registered Practical Nurse with current registration with the CNO.; Bachelor’s Degree with a Social Service Worker Diploma and a current registration with the O.C.S.W.S.S.W; or a post-secondary degree with certification with a relevant professional registration (acceptable to the employer). Experience engaging and working with Indigenous Peoples in a community setting. Experience and a detailed knowledge of serious mental illness, usually gained by several years of working with this population. A demonstrated knowledge, awareness and sensitivity to Indigenous Cultures and Traditions. Valid Ontario Driver’s Licence and access to a vehicle are required. Hours of Work: 35 hours/week.
