The Intake Coordinator serves as the first point of contact for patients referred for mental health services, conducting clinical intake and risk assessments. They coordinate patient entry into programs, manage scheduling, and facilitate referrals to community resources while collaborating with a multidisciplinary team.
Requires a Social Work Diploma or a bachelor's degree in mental health or a related human services field, along with active professional registration. Candidates must have at least 2 years of direct clinical experience with adult mental health patients and certifications in risk identification or de-escalation.

The Intake Coordinator works as part of a multidisciplinary team serving community-based patients within the Mosaic Primary Care Network. The Intake Coordinator reports directly to the Supervisor, Mental Health and plays a key role in creating a seamless and supportive experience for patients being referred to Mosaic for mental health services.
Serving as the first point of contact, this position guides patients through an established clinical intake process, gathers essential information, and addresses questions to ensure clarity and understanding. The role is responsible for collecting and documenting patient data while maintaining strict confidentiality and compliance with organizational and regulatory policy. The Intake Coordinator will facilitate entry into MPCN mental health services – including appointment scheduling – and will coordinate referrals to community mental health and other resources as appropriate.
The Intake Coordinator supports collaborative working relationships with external primary care providers and the MPCN multidisciplinary team and participates in ongoing quality improvement activities, program development, and evaluations projects to support the continued refinement and delivery of high-quality clinical care.
Other responsibilities as required.
Qualifications: & Requirements
• Minimum of 2 years of direct clinical experience working with adult patients with mental health concerns in a mental health focused setting.
