Provide nutritional expertise to patients with nutrition-related conditions through assessment, planning, and implementation of optimal care. Collaborate with multidisciplinary teams and family physicians to deliver individualized treatment plans and population health programs.
Requires a Bachelor's degree in nutrition and licensure with the College of Dietitians of Alberta. A minimum of 3 years of relevant experience in public health or community-based nutrition is required.

The Primary Care Dietitian works within a team-based environment in Mosaic PCN clinics and physician offices. The primary purpose of the role is to provide nutritional expertise to address the needs of patients experiencing nutrition-related conditions/diseases. The Primary Care Dietitian strives to improve health through nutrition strategies consistent with population health promotion. The focus is on achieving outcomes to meet the Mosaic PCN objectives.
The Primary Care Dietitian assesses, plans, implements, and evaluates appropriate and optimal nutritional care with the goal to improve nutritional and health status. The role consults with patients both one on one and in group settings.
Other responsibilities as required.
