Job description

Carefirst Seniors & Community Services Association is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area.

With a staff of more than 550 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are “home-bound” and frail.

We are currently seeking a Community Ambassador to fill a vacant position at our Community Health Team, with a primary focus on supporting our Black Health Initiative. This role plays a key part in advancing health equity by working closely with Black communities to improve access to preventive care, health education, screening, and system navigation.

Carefirst strongly encourages applications from individuals who identify as Black, African, Caribbean, or Afro-descendant, as well as candidates from other equity-deserving groups. Lived experience within Black communities is considered a strong asset for this role.

About the Community Health Team

The Carefirst Community Health Team supports population health by bringing preventive care, health education, screening, and system navigation directly into the community, with a focus on equity and early intervention. • The Community Health Team delivers culturally appropriate, community-driven and equity-focused programs, including:
  • Preventive Care Clinic
  • Cancer Prevention Program
  • Mobile Health Clinics
