The Community Ambassador conducts culturally responsive outreach and education to improve access to preventive healthcare and screenings within Black communities. They connect community members to essential health and social services while assisting in program development and evaluation.
Candidates should have post-secondary education in health promotion or human services, combined with experience in community-based outreach and EDI practices. A valid Ontario Driver's license, access to a vehicle, and fluency in a second language are required or highly desired.
Carefirst Seniors & Community Services Association is a non-profit charitable community services agency established since 1976. Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area.
With a staff of more than 550 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are “home-bound” and frail.
We are currently seeking a Community Ambassador to fill a vacant position at our Community Health Team, with a primary focus on supporting our Black Health Initiative. This role plays a key part in advancing health equity by working closely with Black communities to improve access to preventive care, health education, screening, and system navigation.

The Carefirst Community Health Team supports population health by bringing preventive care, health education, screening, and system navigation directly into the community, with a focus on equity and early intervention. By working alongside community partners, the team helps identify unmet needs, reduce barriers to care, and connect individuals to the right health and social supports at the right time.
These programs aim to improve equitable access to care, promote wellness, and strengthen connections within diverse communities.
Terms: Part Time Contract (From July 1, 2026 to June 30, 2027)
300 Silver Star Blvd., Scarborough and community locations in the GTA
Pay range: $21 – $25 hourly
