The Registered Dietitian will assess patient nutritional needs and develop individualized care plans within an interdisciplinary team. They will also provide diabetes education and support the Chronic Disease Prevention and Maintenance Program through direct service delivery.
Candidates must hold a Bachelor of Science in Nutrition and be registered with the College of Dietitians of Ontario. At least two to three years of relevant experience and strong interpersonal skills are required for this role.
The Carefirst Family Health Team (FHT) is seeking Full-time permanent Registered Dietitian to join our innovative, interdisciplinary team dedicated to delivering high-quality, community-based care.
Carefirst FHT is committed to delivering exceptional, comprehensive primary care through a collaborative, team-based model that includes family physicians, nurse practitioners, nurses, social workers, dieticians, physiotherapists, and medical receptionists. We have a robust Chronic Disease Management and Prevention program, including a Diabetes Education Program, and are actively involved in many of the Ontario Health Team’s primary care initiatives.
Our strong community partnerships ensure coordinated, high-quality, patient‑centred care across the continuum. We work closely with the Carefirst Seniors & Community Services Association to provide truly integrated services.
We currently operate four well-established clinics in Scarborough, Markham and Richmond Hill. Beginning in Fall 2025, we will be expanding in Markham and Stouffville to support continued growth and community needs.
Employees will be assigned to a location that aligns with their preferences and residence, with occasional rotation to support clinic operations.
We are currently seeking a Registered Dietitian to fill a new position at our Family Health Team.
Terms: Full Time Permanent
$65,000 - $75,000 annually
