Occupational Therapist (Family Health Team)

The Carefirst Family Health Team (FHT) is seeking Full-time permanent Occupational Therapist to join our innovative, interprofessional team dedicated to providing high-quality, integrated, community-based primary care.

About Us

Carefirst FHT is committed to delivering exceptional, comprehensive primary care through a collaborative, team-based model that includes family physicians, nurse practitioners, nurses, social workers, service navigators, dieticians, physiotherapists, medical receptionists and administrative team. We have a robust Chronic Disease Prevention and Management program, including a Diabetes Education Program, and are actively involved in many of the Ontario Health Team’s primary care initiatives.

Our “Hub & Spoke Model” and strong community partnerships ensure coordinated, high-quality, patient‑centred care across the continuum. We work closely with Carefirst Seniors & Community Services Association to provide truly integrated services.

• Clinics: psychiatry, cardiology, endocrinology, optometry, geriatric medicine, gynecology, chiropody, rheumatology, physiotherapy, and audiology

Clinic Locations

We currently operate four well-established clinics across Scarborough, Markham and Richmond Hill, and continue to expand in Markham and Stouffville to support continued growth and community needs.

Employees will be assigned to a location that aligns with their preferences and residence, with occasional rotation to support clinic operations.

We are currently seeking a Occupational Therapist (OT) to join our Family Health Team.

Terms: Full Time Permanent

Pay range

$75,000 - $85,000

Responsibilities

• Collaborate closely with the interprofessional primary care team to provide clinical assessments, interventions, recommendations, and maximizing the scope of the OT in a primary care setting. OT contribute to coordination of care and providing patient-centred care to optimize patients’ functional abilities in activities of daily living, safety, cognition, behavioural support, physical function, and access to assistive devices and other resources.
• Contribute to the continuous development, implementation and evaluation of Carefirst’s chronic disease prevention and management programs via the Hub and Spoke model with an equity-based population health lens.
• Participate in ongoing program development and quality improvement activities.
• Prepare personalized care plans for individual patients, and participate in case conferences to monitor and review patient care plans, treatments and program evaluation activities
• Connect patients with appropriate care/support services, and provide educational and community resources to clients and their families
• Maintain accurate records on patient cases, group work/activities, financial reports, and evaluation reports
• Perform administrative and clinic management tasks such as, developing clinical policies and protocols; act as a trainer/mentor to provide in-service orientation, education and skills training to other team members and students; participate in Agency committees (i.e., Infection Prevention & Control Committee, Ethics Committee)
• Contribute to the development of integrated clinical pathways and other development in the local health systems, working closely with Ontario Health Teams (OHTs) such as the Eastern York Region North Durham Ontario Health Team (EYRND OHT)

Requirements

• Master degree in Occupational Therapy or equivalent with minimum of 1 year relevant experience
• Registration with the College of Occupational Therapists of Ontario in good standing
• Member of Canadian Association of Occupational Therapists (CAOT) and/or Ontario Society of Occupational Therapists (OSOT) is preferred
• Experience in psychosocial, cognitive and physical health assessment and intervention to support occupational performance, with experience working across diverse patient populations in various health care settings considered an asset
• Possess ADP authorizer designation and experience with wheelchair and seating assessment / prescription is an asset
• Excellent communication and interpersonal with the ability to build and maintain effective partnerships with internal and external stakeholders through a flexible, collaborative, and proactive approach while working independently and as part of a team.
• Demonstrated proficiency in electronic medical record (EMR) systems and Microsoft Office. Demonstrated interest in digital enablement of care such as AI.
• Committed to fostering a healthy and safe workplace through adherence to organizational policies, applicable health and safety legislation, best practices, and mandatory training requirements.
• Valid vulnerable sector police record check completed with one year
