Collaborate with an interprofessional team to provide clinical assessments and interventions that optimize patients' functional abilities and safety. Contribute to the development of chronic disease prevention programs and maintain accurate patient records and clinical protocols.
Requires a Master's degree in Occupational Therapy and registration in good standing with the College of Occupational Therapists of Ontario. Minimum of one year of relevant experience is required, with ADP authorizer designation and EMR proficiency considered assets.
The Carefirst Family Health Team (FHT) is seeking Full-time permanent Occupational Therapist to join our innovative, interprofessional team dedicated to providing high-quality, integrated, community-based primary care.
Carefirst FHT is committed to delivering exceptional, comprehensive primary care through a collaborative, team-based model that includes family physicians, nurse practitioners, nurses, social workers, service navigators, dieticians, physiotherapists, medical receptionists and administrative team. We have a robust Chronic Disease Prevention and Management program, including a Diabetes Education Program, and are actively involved in many of the Ontario Health Team’s primary care initiatives.
Our “Hub & Spoke Model” and strong community partnerships ensure coordinated, high-quality, patient‑centred care across the continuum. We work closely with Carefirst Seniors & Community Services Association to provide truly integrated services.
We currently operate four well-established clinics across Scarborough, Markham and Richmond Hill, and continue to expand in Markham and Stouffville to support continued growth and community needs.
Employees will be assigned to a location that aligns with their preferences and residence, with occasional rotation to support clinic operations.
We are currently seeking a Occupational Therapist (OT) to join our Family Health Team.
