The Health and Wellness Worker assesses client health barriers and develops individualized wellness plans for individuals facing homelessness and systemic marginalization. They coordinate access to clinical care and external community resources while collaborating with internal teams to ensure integrated service delivery.
Candidates need a college diploma or university degree and at least 3 years of experience in health or substance use support, including 2 years in a shelter setting. Strong knowledge of Toronto's community health resources and proficiency in MS Office are required.

The Health and Wellness Worker plays a key role in supporting clients who face multiple barriers to optimal physical, mental, and emotional health — including experiences of homelessness, trauma, substance use, and systemic marginalization. In this role, you will work directly with clients to assess their health and wellness needs, support the development of individualized goals and plans, and coordinate access to internal programs and external services.
You will collaborate with team members across CONC’s programs — including Life Management, Employment, and Sustainable Housing — and contribute to the development and delivery of creative, inclusive, and community-driven health strategies. This role requires strong knowledge of Toronto’s health and community resources and a deep commitment to client-centered, anti-oppressive practice.
This is a unionized, full-time position reporting to the Supervisor of Shelter Programs. Collaboration with the Managers of Drop-In, Men’s Shelter, and Transitional Housing Programs is also essential.
