Position Profile This position is assigned to a housing program located at 1011 Lansdowne Avenue. Unlike some Follow-Up Housing Support Worker positions that operate on a community-based model, this role is primarily site-based and provides on-site support to residents living within the building.
The successful candidate will work closely with tenants who have experienced homelessness and may present with complex and high-acuity needs. The role requires on-site engagement, crisis intervention, housing stabilization support, service coordination, and tenancy support to help residents maintain successful housing outcomes. This role is designed to promote long-term housing stability by offering individualized support in the community, including crisis intervention, harm reduction, life skills training, and connection to key health and social services.
You will work with individuals facing complex barriers such as mental health challenges, trauma, substance use, immigration issues, and experiences within the criminal justice system. The ideal candidate is compassionate, proactive, and committed to empowering individuals toward greater independence.
This is a unionized position reporting to the Supervisor of Housing Follow-Up Support.
