The Occupational Therapist assesses individual needs and implements programs focusing on discharge planning and community liaison. They collaborate with clients, families, and multidisciplinary teams within a Satellite Health Facility.
Candidates must be registered with the College of Occupational Therapists of Ontario and possess thorough knowledge of OT skills. Proficiency in functional (COPM) and cognitive (CCT) assessments is required.
As a member of the Satellite Health Facility, the Occupational Therapist will, in collaboration with the client, family and multidisciplinary team, assess individual needs, plan, and implement programs for individuals with a major focus on discharge planning and community liaison. The SHF operates as part of the Community and Population Health Program and is approved to conclude on or before March 31, 2027. Occupational Therapists (OTs) recruited for the SHF are hired into permanent positions through the Nursing Resource Team (NRT).
CSW2626-X2: Alternating Friday and Saturday: 10pm to 9am
CSW2626-E2: Alternating Saturdays and Sundays: 9am to 4pm
