Since 1973, The Alex has been a community-based, not-for-profit organization providing integrated primary health care, community support services, and housing-focused programs to individuals and families who experience barriers to accessing traditional health, housing, and social service systems. Services are grounded in principles of health equity, low-barrier access, trauma-informed practice, cultural safety, harm reduction, and person-centered care.
HomeBase is a scattered-site Recovery Oriented Supportive Housing program operating through an Intensive Case Management (ICM) model. Participants live independently in apartments throughout the community and receive individualized, flexible supports focused on housing stability, health coordination, system navigation, income and benefits support, crisis intervention, and community integration.
Reporting to the Program Manager, The Registered Nurse (RN) is a key member of a multidisciplinary Intensive Case Management (ICM) outreach team supporting individuals experiencing complex health, mental health, substance use, and housing instability challenges. Working within a Housing First and Harm Reduction framework, the RN provides nursing assessment, health education, advocacy, care coordination, health system navigation, and clinical consultation. The RN collaborates with case managers, healthcare providers, and community partners to support health stability, housing retention, and community integration. Working to full scope within a trauma-informed, participant-centered, recovery-oriented, and strength-based model, the RN applies independent clinical judgment, assessment, care planning, intervention, consultation, and escalation as required to improve health outcomes and facilitate access to community-based supports.
This is a full-time, permanent position.

This role operates in high-stress and unpredictable community environments where exposure to medical distress, substance use, violence, trauma, grief, and mental health crises may occur. The ability to maintain composure, apply de-escalation strategies, and exercise sound clinical judgment is essential.
