Registered Nurse

• Case Management and Care Coordination
• Work to full scope of RN practice, including independent assessment, clinical decision-making, participant education, care coordination, risk identification, and appropriate escalation within professional, organizational, and legislative standards.
• Complete nursing assessments and identify emerging or changing health concerns, including physical health, mental health, substance use, medication-related concerns, functional needs, and risks impacting housing stability.
• Support participant navigation and access to primary care, addiction medicine, mental health services, and community resources both within The Alex and in the broader community.
• Collaborate with interdisciplinary team members to develop, implement, and regularly update participant-centered service plans, health goals, and safety plans, with a primary focus on supporting health stability and its impact on housing stability and community integration.
• Participate in case conferencing, care planning meetings, and interdisciplinary consultations to support coordinated care.
• Coordinate care with internal and external providers to ensure continuity of services.
• Support transitions between hospital, detox, shelter, treatment, and community settings.
• Identify barriers to engagement and service access and work collaboratively to address them.
• Identify risks to health and housing stability and collaborate with participants and team members to address them.
• Advocate for participant rights and equitable access to health, housing, income, and social supports.
• Maintain strong relationships with community partners to support integrated service delivery.

• Administrative Responsibilities
• Complete documentation, assessments, reporting, and data entry in accordance with organizational, professional, and legislative requirements.
• Maintain accurate, timely, and high-quality clinical records and case notes.
• Track nursing activities, referrals, and follow-up to ensure continuity of care.
• Participate in quality improvement, evaluation activities, and ongoing professional development.

• Team Responsibilities
• Work collaboratively as an active member of a multidisciplinary outreach team.
• Demonstrate professionalism, flexibility, initiative, and sound clinical judgment in both independent and team-based work.
• Contribute to a positive, communicative, and collaborative team culture.
• Participate in team meetings, case consultations, and training opportunities.
• Provide consultation and support to colleagues regarding nursing practice and participant care.
• Perform additional duties as assigned.

• What You Bring
• Graduate of an accredited degree program in nursing.
• Active practice permit with the College of Registered Nurses of Alberta (CRNA).
• 3-5 years recent experience in a primary care setting.
• Experience working with individuals experiencing homelessness, poverty, mental health concerns, and substance use disorders.
• Class 5 driver’s license, personal vehicle for business use, clean 3-year drivers abstract, and $2 million liability coverage are required.
• Minimum of 21 years of age and three (3) years of driving experience is required to drive an Alex fleet vehicle.
• Basic Life Support (BLS) within 30 days of hire date; certification must be valid for a minimum of 6 months upon hire.
• A satisfactory vulnerable sector criminal record clearance is required.
• Demonstrated knowledge, understanding and commitment to the Truth and Reconciliation Commission of Canada: Calls to Action is preferred.

• Work Environment and Hours
• Work is primarily conducted at The Alex Community Health Centre: 2840-2 Ave SE, Calgary, AB; however, some travel is the community may be required.
• Standard Hours: 8:30 am – 4:30 pm, Monday to Friday; some scheduling flexibility may be available and required.
• FTE 1.0 (37.5 hours/week)
• Requires extended periods of sitting, with some standing and walking (sometimes up and down stairs).
• Involves physical action requiring the ability to bend/crouch and reach above shoulder height.
• Outreach or home visits as needed.
• Occasional driving may be required as part of work duties.
• Occasional time outside in all weather conditions.

Driving on Behalf of The Alex

• This position requires the use of a personal vehicle for work-related purposes and driving an Alex fleet vehicle.
• Successful candidates must show proof of a class 5 driver's license, a personal vehicle, and a clean 3-year drivers abstract.
• Candidates must consent to the possibility of an annual driver's abstract review to meet our insurance provider’s requirements and to maintain $2 million liability insurance on their personal vehicle.
• Driving a fleet vehicle at The Alex requires that staff are at least 21 years of age with a minimum of three (3) years’ driving experience.
• Mileage will be reimbursed in alignment with the CRA guidelines.
