Provide comprehensive clinical care and follow-up for children aged 0-18 within a multidisciplinary healthcare team. Perform therapeutic roles including assessment, counseling, and advocacy for families dealing with acute and chronic medical illnesses.
Requires a Master's Degree in Clinical Social Work and registration with the Ontario College of Social Workers and Social Service Workers. Candidates need 3-5 years of clinical experience with children and families, specifically regarding medical illness and bereavement.
As a member of the multidisciplinary healthcare team, the Social Worker will apply their clinical knowledge and skills to provide comprehensive care, and follow-up to children ages 0-18.
Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers as a Social Worker. Master's Degree in Clinical Social Work from an accredited School of Social Work 3-5 years clinical experience (within the last 6 years) with children and families dealing with acute and chronic medical illness, death and dying, bereavement issues and chronic progressive illness Hospital experience is preferred Excellent knowledge of community support and placement services Knowledge of Healthcare Consent Act, Substitute Decisions Act and Child and Family Services Act. Proven ability to work as a member of interdisciplinary healthcare teams. Demonstrated skills in the performance of a therapeutic role including strong assessment, counselling, case management and advocacy skills Proven ability to work in a fast-paced, complex environment and to organize, prioritize and problem solve effectively Demonstrated commitment to ongoing learning Strong interpersonal, communication, teamwork, consultation and teaching skills
