Position summary

Under the direction of the Client Services Manager, this position is responsible for connecting tenants with available resources in the community to assist with their transition back into independent living safely. The Life Skills Support Worker arranges structured assistance by networking with community supports to the tenant to help develop and maintain life skills related to time management, money management, personal hygiene, household hygiene, organizational and interpersonal communication, menu planning, grocery shopping, meal preparation and community access. This position will collaborate with the tenant to develop a service plan with specific goals and interventions for the transition back into community living with community supports and assistance. KEY ACCOUNTABILITIES (INCLUDE BUT NOT LIMITED TO):

• Under the direction of the Client Services Manager and working as a member of the interdisciplinary team, the Life skills Worker:
• will be responsible for conducting assessments, determine service goals and develop services plans in collaboration with tenants and site staff members
• will be responsible to facilitate tenant connections with community and government supports and advocate for tenants with these external service providers
• will be responsible to focus on establishing housing, financial, food and clothing, transportation, and social supports for tenants
• will be responsible for providing education and coaching support related to daily living skills and activities (attending appointments, money management, searching for jobs, grocery shopping, etc.)
• assists residents with mental, developmental, and/or physical disabilities to live successfully in the site
• engages the tenant and their support systems as people with full competence as full collaborators in service planning and evaluation.
• develops a transition plan for re-integration back into the community in collaboration with the tenant and other departments
