Coordinate support networks and care plans for seniors with serious mental illness and addictions to increase their independence and dignity. Provide direct services including crisis intervention, supportive counselling, and linkages to primary healthcare within the community.
Requires a diploma or degree in Social or Human Services with at least three years of experience supporting vulnerable seniors with mental health or neurological conditions. Must possess a valid driver's license, a reliable vehicle, and the ability to pass a vulnerable sector check.

As a Psychogeriatric Case Manager, you will be part of a team that co-ordinates a support network and linkages for older adult and seniors with serious mental illness and other challenges (e.g., addictions). A key component of this position is building a support network of community resources that will enable the individual to increase their independence, dignity, and self-respect. The role will require direct service and support to clients in area such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs.
