Candidates must have a bachelor's level education in Social Work or Social Science and at least 3 years of experience in community mental health and addictions. A valid driver's license and the ability to work with diverse communities are also required.

The Role and Opportunity
CMHA Toronto’s Mental Health and Justice Case Management Program is seeking a Site based Case Manager. The Case Manager will work primary at a residential site, within team of case managers primary in the West (sometimes East) end of Toronto to provide residential/community-based support and linkages to individuals with serious mental illness who are at significant risk of incurring criminal charges and/or at significant risk for apprehension by the police.
Salary: $54,943 – $63,988
