Core responsibilities of the position

• The Nurse Practitioner (NP) provides autonomous comprehensive primary care to individuals and families across their lifespan. The focus is on health equity, chronic disease management, prevention and care for the underserved populations. Working within an interprofessional team, the NP delivers patient-centered care aligned with the values of community health and social determinants of health.
• Perform full scope assessment, diagnosis and treatment, health education/counselling and other clinical activities according to the College of Nurses of Ontario standards of practice for Registered Nurses in the Extended Class (NPs) as well as the Medical Directives of weCHC
• Carry own roster of clients/patients as the primary care provider
• Order and interpret diagnostic tests (labs, imaging)
