Overview

Provide client-centered social work services as a member of a multidisciplinary health care team. Assess clients and identify social, economic, emotional, or psychological barriers to care and/or optimum level of function. Develop and implement care plans. Provide intervention, consultation, and referral for services to clients and their caregivers/families. Provide counseling including crisis, loss and grief counseling. Educate and consult clients/caregivers/families, support groups, and other health care professionals. Develop and coordinate inservices and presentations and facilitate sessions. Determine and deal with emergent situations. Organize, prioritize, and coordinate workload in a complex multi-site environment. Communicate effectively verbally and in writing. Establish and maintain effective working relationships with clients, staff, and other health care providers. Work independently and as part of, and/or provide leadership to, a multidisciplinary health care team. Operate related equipment including computerized systems

Overview

Plans, implements, and evaluates medical nutritional therapy for hospital-based and/or facility-based patients according to Nutrition Services guidelines. Instructs and counsels patients and families regarding appropriate nutritional requirements, nutritional regimes, and food planning and preparation. Coordinates activities and liaises with medical and nursing staff, patients, and the food service department and/or support agencies
