Overview

Provide service coordination and urgent response as required. Utilize specialized treatment modalities/therapies for individuals, groups, and/or families. Arrange access to specialized services (substance use, forensic/criminal justice, developmental disability, acquired brain injury). Ensure transition plans are in place for clients transferring between levels of care, including admission to acute care and discharge planning/follow-up. Document assessments, treatment formulations, care plans, and progress notes. Consult and share information with community agencies and the public about mental illness and mental health/substance use services. Deliver and participate in in-service and other educational programs as required. Perform other related duties as assigned

Provide timely, responsive, evidence-based treatment and clinical supports across the continuum of available services for clients with serious severe and persistent mental health problems and/or problematic substance use and/or development disability issues who may or may not be in crisis. Provide assessment, treatment, case management, community support, and urgent response as required. Function as a primary clinician using specialized treatment modalities/therapies for individuals, groups of clients, and/or their families. Support initial intake and assessment for clients interested in the Opiate Agonist program. Work with physicians and community services to ensure therapy initiation, support, streamlined transition, and continuity of care. Coordinate overdose prevention education.
