Perform professional social work of moderate complexity in a hospital setting. Assist patients and their families with practical problems and personal concerns. Consult with hospital staff and community agencies regarding psychosocial aspects of patients' situations. Provide short-term supportive counselling related to grief, loss, adjustment to diagnosis or change in health status. Support complex discharge planning with clients and their families to return to home community or alternate level of care.
