Job Description

In accordance with the Vision, Purpose, and Values, and strategic direction of the Island Health (Vancouver Island Health Authority), patient & staff safety is a priority and a responsibility shared by everyone at VIHA; as such, the requirement to continuously improve quality and safety is inherent in all aspects of this position. Reporting to the Clinical Coordinator, performs professional social work of moderate complexity. Assists patients and their families with practical problems and personal concerns and consults with hospital staff and community agencies regarding the psychosocial aspects of the patients' situation.

Qualifications

• Education, Training And Experience Bachelor of Social Work degree (B.S.W.) from an accredited school of Social Work, however a Master's degree (M.S.W.) is preferred. Current full registration with the BC College of Social Workers. Related social work experience, preferably in a medical setting, and demonstrated clinical competence. Skills And Abilities
• Ability to use and exercise judgment as required.
• Ability to work independently with consultation and guidance available.
• Knowledge of community resources.
• Demonstrated effective oral and written communication skills.

Previous experience in critical care and specifically cardiac care will be an asset. The social worker works in team-based care. Advance practice skills in short-term counselling services specifically in adjustment to change of health status, grief , loss and end of life are part of the social work role. The social worker explores all aspects of Advance Care planning with the client and their family. The social worker engages in complex discharge planning with the client and their family to return to the community or to an alternate level of care. Knowledge of community and government, financial and housing resources will be integral to supporting discharge planning. All social workers are Designated Responders under the Adult Guardianship Act and must investigate, assess and intervene in allegations of Adult Abuse, neglect and self-neglect. Social Workers collaborate with the Ministry of Children and Families on all matters related to child protection.
