Travel LPN - Long Term Care (Evening Shift)
The LPN will provide chronic disease management, administer medications, and perform routine clinical procedures for residents in a long-term care setting. They are also responsible for monitoring vital signs, documenting patient responses in the EHR, and coordinating care with the multidisciplinary team.
Current LPN license in the state of New Hampshire (or an active Compact/NLC license).
Current CPR / BLS certification.
1+ years of experience in long-term care or skilled nursing facilities.
Strong communication skills with families and team members, adaptability, and proficiency in EHR documentation and care planning.
Hands-on experience with wound care management or palliative care principles.
