The LPN will provide person-centered care, manage chronic diseases, and perform daily assessments for residents in a long-term care facility. They are also responsible for administering medications, performing wound care, and documenting resident responses in the EHR.
We are seeking an experienced and adaptable Travel Licensed Practical Nurse (LPN) for a Long-Term Care (LTC) facility in Dubuque, IA. In this role, you will deliver high-quality, person-centered care, chronic disease management, and daily assessments for residents in a supportive facility environment. Key Responsibilities Monitor vital signs, administer medications, and accurately document resident responses in the EHR. Perform routine procedures, wound care, and dressing changes following established care plans. Identify and report changes in resident condition to RNs, physicians, and family members. Support activities of daily living (ADLs) and rehabilitation plans in coordination with CNAs and therapists. Maintain strict safety protocols and uphold state and facility compliance standards.
Education: Graduate of an accredited Practical Nursing program.
Active LPN license in the State of Iowa (or active Compact/NLC state license).
Active CPR / BLS certification.
1+ years of experience in long-term care or skilled nursing facilities.
Willingness to work rotating shifts (Days, Evenings, and Nights).
Strong communication with residents' families, multidisciplinary teams, and proficiency with EHR systems.
