Nurse (Health and Wellness Coach) with Mental Health and Addictions Experience

Job description

We’re looking for an excellent nurse with experience working with people with mental health and addictions. Collaborating as part of an integrated, multi-professional Coaching Resource Team you will contribute to the development and implementation of individualized Life Redesign Plans for the people we support after brain injury and stroke. You will participate in the mentoring and development of staff, contribute to our continuous learning program, and help us in building upon our standards for excellence, and our commitment to continuous quality improvement.

You will report directly to the Leader - CONNECT Hamilton.

  • Can bring their skills as a nurse (with registration in good standing with the College of Nurses of Ontario) to the table;
  • Is comfortable working in a flexibly structured, intentionally different culture;
  • Prefers guidelines to rules, grey versus black and white;
  • Will contribute creatively to the development of services drawing from their unique strengths and interests;
  • Can juggle multiple priorities and still maintain a sense of perspective and fun.

What the role looks like. Within your scope of practice

Guided by the individual’s priorities and goals, the Health and Wellness Coach contributes to the creation of an integrated, multi-professional Life Redesign Plan by supporting the generation of solutions to address factors impacting the person’s goals and plan for Life Redesign. The Health and Wellness Coach serves as a nurse, coach, mentor and teacher for the people we support, families, and co-workers, informs the development of appropriate support strategies for individuals and staff, providing nursing skills as needed for the individual’s health care needs and lifestyle choices.

The Health and Wellness Coach contributes to CONNECT’s orientation and continuous learning, and helps build on standards for excellence, and commitment to continuous quality improvement.

Requirements

Candidates must hold a valid nursing license with the College of Nurses of Ontario and possess experience working with individuals facing mental health and addiction challenges. The role requires the ability to work in a flexible, multi-professional environment and a commitment to improving the lives of those supported.

Health and Wellness Coaches work as part of an integrated team with coaches from other disciplines and Life Redesign Coaches (support staff), who support people in their homes. Life Redesign Coaches are people with diverse backgrounds whose collective strengths offer a rich resource for supporting people and their work towards life redesign.
