Case Manager – New Dimensions Assertive Community Treatment Team (ACTT) – Full-Time, 4 Month Contract
The Canadian Mental Health Association (CMHA) – Toronto is seeking a Case Manager for our New Dimensions Assertive Community Treatment Team (ACTT). Based at the Markham site, this multidisciplinary team provides community-based support, rehabilitation, and treatment to individuals living with mental illness. The ACT Team promotes stabilization, community integration, recovery, and overall quality of life through client-centered services delivered directly in the community.
Hiring Range: $54,943 – $60,203 + 4% in lieu of vacation
Hours of Work: 35 per week

Candidates must have a post-secondary degree or diploma in social services or a related field. A minimum of three years of direct service experience supporting adults with psychiatric disabilities and a valid Ontario driver's license are required.
